Baptism Form

“So go and make followers of all people in the world. Baptize them in the name of the Father and the Son and the Holy Spirit.” – Matthew 28.19

Baptism is one of the two ordinances of the Christian faith, instituted by Jesus Christ as a sacred act of obedience and public declaration. Along with Holy Communion, it stands as a foundational expression of our faith and commitment to God. Baptism symbolizes the believer’s identification with Christ’s death, burial, resurrection, and ascension—going under the water represents the old life being buried, rising from the water reflects new life in Christ, and walking out of the water signifies the continuation of telling and showing others your walk with Christ as you live empowered by the Holy Spirit.

This is a special and celebratory day, marking a powerful spiritual milestone in a believer’s journey. It is an outward demonstration of an inward transformation—a public testimony that one has accepted Jesus Christ as Lord and Savior and has chosen to follow Him.
